Overview

Job Summary

Under the supervision of the Petersburg Program Manager, the Youth Transitional and Rapid Re-Housing (RRH) Case Manager will support the Youth Housing Demonstration Program (YHDP), which helps young adults (ages 18–24) transition out of homelessness into safe, stable, and permanent housing.

This role works collaboratively within a team-based, systems-focused model that aligns with St. Joseph’s Villa’s mission, values, and guiding principles. The Case Manager will also coordinate with Flagler Leadership staff to develop, implement, and maintain program effectiveness.

 

Responsibilities

Essential Job Functions

  • Provide support and housing planning to youth households temporarily staying in hotel-based transitional housing until permanent housing is secured.

  • Identify eligible youth households for transitional housing intervention in line with funder and program requirements.

  • Build and maintain professional relationships with hotel staff to support program participants.

  • Conduct assessments and develop individualized stabilization plans, coordinating temporary housing as needed.

  • Assist youth with obtaining and maintaining rental housing, including support with deposits, furniture, and donations when appropriate.

  • Deliver case management services, including referrals, collaboration with community partners, and life skills/educational support.

  • Work with community resources to help clients access education, employment, and supportive services.

  • Process financial assistance applications and check requests for subsidies and related expenses.

  • Maintain accurate documentation in paper files and the HMIS (Homeless Management Information System) in compliance with funder and regulatory requirements.

  • Share client progress and seek guidance on high-barrier cases during weekly case coordination meetings.

  • Participate actively in team meetings, community events, and collaborative activities essential to program operations.

  • Perform additional duties as assigned.

Qualifications

Qualifications

  • Education: Bachelor’s degree in a human services field preferred (relevant experience may substitute).

  • Experience: Minimum of 1 year working directly with a similar population.

  • Proficiency with MS Office Suite (Word, Excel, PowerPoint, Outlook) and internet research.

  • Valid Virginia driver’s license and excellent driving record.

  • Fluency in Spanish preferred.
