Overview

Under the direction and guidance of the Director of Community Based Services and the Clinical Supervisor, delivers treatment services to program clients and their families within the policies and procedures of St. Joseph’s Villa. Responsible for the coordination of an overall written program of services; coordination of program staff in relationship to Service Plans for clients and providing direct professional services to clients and their families.

Position focuses upon development of and participation in a team approach with other program staff and community-based support to insure quality services to clients and their families. Performs all work safely, in accordance with the highest professional ethical standards, and in alignment with Saint Joseph’s Villa mission, values, and guiding principles.

 

Responsibilities

 

Participates as an active member in a team approach to the delivery of services to clients.

Participates in staff meetings, community meetings, and other staff and program group activities that support the operation of an effective psychoeducational program.

Coordinates all phases of the treatment program within the program(s) for those clients for whom he/she has primary responsibility.

Participates in the development and implementation of service plans, objectives and methods necessary for providing effective services to individual clients and families.

Provides professional services to clients and their families in the family’s home or in appropriate community activities on a regularly scheduled basis.

Develops of progress/evaluation reports on clients that meet both St. Joseph's Villa’s and Medicaid’s requirements.

Maintains accurate individual case records for clients under his/her supervision within state licensing and professional accreditation standards.

Promotes good community relations and utilizes needed community services.

Acts as liaison with community agency casework staff for process reporting and service planning.

Assists in client intake, admission and discharge planning.

Provides after-care services for discharged clients and their families to the extent possible.

Evaluates work processes and methods and submits recommendations for continuous quality improvement.

Participates in weekly face to face clinical supervision.

Attends scheduled training programs for professional development that includes, at a minimum, trainings required by St. Joseph’s Villa and by regulatory and accrediting bodies.

Performs other duties as assigned.

Qualifications

BA/BS degree in Human Services. One (1) or more years related experience, Master’s degree preferred - Qualified Mental Health Professional-C (QMHP-C). Proficient with MSOffice (Outlook, Word, Excel and PowerPoint). Valid Virginia driver’s license and excellent driving record preferred.
