Under the direction and supervision of the CSS Clinician, Program Manager, or Program Director, the Clinical Case Manager coordinates the overall services provided to clients with external agencies and maintains accurate outcome data for the service. All work is performed within a systems-based framework and in a psycho-educational model. Work is performed collaboratively with and in support of other services offered by St. Joseph's Villa.


All work is performed safely, effectively, to the highest ethical standards, and in accordance with regulatory and accrediting standards and St. Joseph’s Villa’s mission, values, and guiding principles.


  • Provides intensive case coordination using the High Fidelity Wraparound model for clients and families in coordination with Community Service Boards and other relevant parties (schools, physicians, private providers) to support stabilization upon discharge.  Works collaboratively with the CSU team. 

  • Conducts mental health assessments to determine eligibility for services.  Acts as a liaison between tele psychiatry psychiatrist and crisis stabilization team in order to ensure continuity of care. 


  • Works with treatment team to develop action plans and crisis prevention plans based on the family’s prioritized needs.


  • Works directly with and in conjunction with treatment team to identify client needs and most effective treatment for continuous stabilization upon discharge. 


  • Researches community-based organizations and governmental agencies to determine what resources are available to support the client’s needs.  Actively links clients and their families to these identified resources.


  • Maintains case management records. Compiles information to prepare discharge summary reports on clients. Maintains contact with designated family members or guardians and school systems. Coordinates community support and intra-agency support as needed for each individual client.


  • Maintains smooth processes and coordination for the tele-medicine clinic to include scheduling, maintaining equipment, preparing documents for medical staff, etc.


  • Administers medication, oversees compliance with medication logs, and storage of medications.


  • Prepares regular narrative, tabular, and statistical reports for St. Joseph’s Villa management staff, Service Board, and licensing, accrediting, and regulatory agencies.


  • Prepares data for accurate billing for services, complying with the requirements of the varied funding streams.Uses approved behavior management techniques to work with clients in crisis.


  • Provides direct client care.


  • Models high level of professionalism with professionals to include behavior and dress.


  • Participates in staff meetings and other staff and program group activities, including after school and evening programs, essential for the operation of an effective psycho-educational program.


  • Evaluates work processes and methods and submits recommendations for continuous quality improvement.


  • Attends scheduled training programs for professional development that includes, at a minimum, trainings required by St. Joseph’s Villa and by regulatory and accrediting bodies.


Performs other duties as assigned.


Participates in the on call rotation.



  • BS/BA degree in Psychology, Human Services, or related field.
  • Two years related experience.
  • QMHP-C required 
  •  Proficient with Windows, Outlook, MS Office including Word, Excel and PowerPoint. 
  • Experience with proprietary software preferred.  
  • Valid Virginia driver’s license with excellent driving record.
